how to choose solver

I am studying blood flow in vessel. Which solver should I choose? Any similar case in tutorials?
I have successfully generated mesh via snappy and it works fine replacing motorbike. So the fluid is actually outside the vessel. I dont understand why there needs a background mesh according to http://www.hydroniumion.de/general/s...mesh-tutorial/ The following thread seems promisingly helpful.
